Terminology issue found on May 6, 2010:

We misnamed one of our code constructions. In section 3.C we
describe the "HD-Combination" code construction. The code
construction is based on the desired Minimum Hamming
Distance of the code. Using just the acronym HD is thus
confusing. We now refer to these code constructions as
Minimum Distance (MD) Combination codes.

Thanks to our colleague Vinay Deolalikar for helping us
clarify our terminology.
